Forms FOR ST. JOSEPH's YOUTH MINISTRY

Overview: All youth within the St. Joseph's Youth Program (both High School and Junior High) are asked to register with the Youth Ministry Office. This allows the Program Director and his staff to have information about the youth, including his or her insurance information and emergency phone numbers on hand.

Registration is done once at the beginning of each year due to th nature of the program - please note that if any vital information changes (i.e. insurance, home phone, etc.). We ask for a $30.00 donation payable to St. Joseph's Church if your means allow upon registering your teenager. All proceeds go towards the future ministry year to cover the costs of food, retreats, shirts, and social events. Overview: On certain off-site and overnight activities, youth must download the following permission form and have it signed and completed by a parent/guardian before attending. Make sure all information is filled out to the best of your knowledge.
